issue with forms

I have a dog welcome packet made for clients and it was working fine. I cloned the forms to make some minor changes and adapt it to cat clients. Now when I try to share the form it is only sharing an old form that has my name as the client?  

https://form.jotform.com/201405563899160 this is the form that is being shared which i do not want this form shared (it has my name and info on it)? the correct form I am trying to share is below. Can you please tell me how to correct this?

Hi!

Your name is showing up on the form link because you have saved and continue enabled on the form. You have certainly tested the form on your browser while login, and now when you access the link it remembers your previous session. 

Your clients will not see your name on the form when you share it with them.  To test this, kindly open the form on a different browser or incognito mode. It should not show your name. .
