WhenI embed my form to our Weebly website it has my email address and name auto-filled into it. How do I clear that out?

Hi Shannon!
This happens because in the form's Settings>Show More Options, the very last setting for Allow Browser AutoComplete is set to Yes:
You are only seeing this because you are logged into your JotForm account. I see my email pre-filled as well. If you log out of your JotForm account and re-load the page, the fields in question should be blank. Your users will not see these fields pre-filled unless they access the form the second time, and then it will have only their information.
You can always set the setting to No if you would like.
