I am not sure if I have understood your requirement. Have you downloaded your form's source code to implement some changes?

If yes, you will have to host this yourself. You need to upload this source code to your website and use it with direct link or embed it in any webpage.

Please let us know if you meant to ask something else.

    Thanks,

I checked your account and both the forms you have created in your account. None of your form field matches with the screenshot you have given above.

In order to receive the notification emails, form must be created in your JotForm account will all the form fields. Once you create the form, you can download the source code and modify it as per your requirement with custom code. You should not change (add / remove) any form fields.

Hope this gives you an idea. Should you have any questions please feel free to contact us.

    Thanks,

I guess, what you are trying to do is to import a form you made using an desktop software right? You cannot do that.

Jotform can only import html forms. Html forms begin with a <form> tag and has form elements that begin with the <input> tag.

I forgot to mention that  looking into FORM TEMPLATES GALLERY to something similar to what you did could be another option
