having a very hard time creating forms

    Good Afternoon, 

    I hope that you can help me. Getting support by email is a bit frustrating - I am hoping that there is a live way that we can do this. 

    I want the form to look more like the sample jpg that i have uploaded including the initial lines inline with text, bolding, etc. 

    I would appreciate a response as soon as possible as I want to confirm that your solution works for me before I take advantage of the discount which expires today.

    Hello!

    I confirm the online form can look like the imported PDF document, but you have to manually customize the fields and map them to the corresponding form fields on the original Doc. 

    You use the fill-in-the-blank element to achieve this.
