Buttons SAVE and NEXT missing from form

Hello! The order request form I’ve made is not publishing properly. When I open it in a new page the layout only shows the first page and has no next page button etc.

I’m hoping someone can help fix this!

I checked your form and found that there are broken conditional rules.

Please open your form's conditions page and remove them.

This will bring your SAVE / SEND and paginations buttons back.
