On one of my forms, I am getting to a blank page where only the background is shown

When I open the form and select "curbside covid testing" it asks if I have an appointment.

When users select NO, I would like to tell them they need an appointment and provide a link to my other form. I do not want them to be able to complete submission on this form as submissions go to our patient queue. 

I tried inserting a paragraph with a link, but this results in a blank page.

I tried inserting an image with a link embedded and it goes to the blank page as well.

I've looked over and over on conditions to make sure I'm not missing anything and I have even tried to skip to that particular frame but it just shows the blank background.

I need to resolve this issue urgently so I can publish my form! We are covid responders

It seems that there is a conflict in your conditions.

However, in order to meet your requirements, I edited your form and created a new Single Choice field. This field will allow you to prevent the user to proceed and submit the form, if they choose "No" to the second question.

Hi, I tried cloning, but it is still saying I am unauthorized....I only need it to have this function on "CURBSIDE COVID Testing - Do you have a curbside appointment - NO" all other functions of my form should stay the same.

This may be related to the HIPAA compliance, thus we will be unable to log in to your account to modify your form. 

I can see your form does not have the condition and element you want, please add the text with the hyperlink and the corresponding condition, let us know once you have done this so we can perform some tests on our end in order to replicate the same issue you have. 

I've added the hyperlink and the single choice with "okay" and the condition to skip back to "please select your type of visit"  but for some reason, no matter what type of question or widget I add in its place it gives me the same exact problem. I keep reading all my conditions to find where I may have caused this but I just dont see anything

I tested your form again. It seems fine now.

"We're sorry, Curbside COVID testing..." field appears when I choose "No" to the second question. I would also recommend you to hide this field ("We're sorry...") and then use conditions to show it, only when it is necessary.

If the issue still persists, please go through each of your conditions and make sure that they do not hide the above field.
