Hi There
As a Soft Tissue Therapist, I require a signed Covid Screening document. I have uploaded the original PDF from my association and managed to create a form. But when I download the submitted form as an PDF I can't open it. It tells me the Pdf file has been damaged and can't be repaired! What is happening?

I have tested a cloned version of your fillable form and it seems to be working at my end.
I have cleared your form cache.

Great that’s better. Though I think I’ve just deleted my forms arrgghhh. Anyhow. Might have start with a new original pdf as one page went missing when I converted the original word document to pdf. Think I’m starting to get hang of it but my guess is I still need a bit of help working this out. The app doesn’t seem to play with me now on my iPad.

Are you importing the Word document into the PDF Converter to create a fillable PDF?
Unfortunately, you cannot import the Word document, it has to have a Fillable PDF to be successfully transformed into an online form.
