Hello,
I completed the integration of google drive with my jotform account. I sent myself a sample form- I signed it, and it says it was sent; however, I cannot find it on google drive.
Where exactly do the forms go after they are sent to google drive?

Hi drsera,
Right before completing the integration, make sure that you set a folder name where all submissions belonging to your form will reside. It will be "Form" by default:

"I have completed those steps. The forms are sent and signed successfully. I can see them in the jotform website under submissions, but it seems as they are not saving to drive."
To which form is your issue related? The only form (Medical Intake Form) on your account that has submissions, in not integrated with Google Drive. If the issue is related to this form, please integrate it with Google Drive and make a test submission to see if the data will be sent to Google Drive. If the issue is related to some other form, please provide us a form URL.

Thank you. Now they are saving, but the title of each form is not being saved correctly. Now, within the folder I created- "Medical Intake Forms"
There are separate folders, but they are named {type A} rather than patients name and date seen....How do I fix it so they are all automatically saved and labeled the same way?

Upon checking, your "Print Name/Signature" field id is {printNamesignature5}, so {typeA} won't capture any value, but instead replace it with {printNamesignature5}.
To fix this, remove the integration and reintegrate it once again and make sure to properly assign the field's unique name.

I have reintegrated it multiple times and it keeps saving as {typeA}...
I just want it to save with the patients name and date. There is no option for me to even choose name/ date when i re-integrate.
Please, how do I fix this? I have been trying for days now.

Your Medical Intake form has 2 Name fields. The unique name of the first "Name" field is {name21} and the unique name of the date field is {date}.
How to Find Field IDs and Names
I tested the Integration on a cloned form and I was able to successfully and the Name and Date fields as the subfolder.
Please remove the integration, integrate the form again, select the Name and Date fields as the subfolder, and make a test submission.
