404 error on exisitng page in Wordpress

    I've got a couple problems with one of my forms nad I was hoping you could help.
    I've used these forms sucessfully for a while now with no issues on my website, however I'm transitioning to a Wordpress site which loves to cause havoc with accessing external sources.
    The page for my testing site is: http://www.chiseledimages.com/misc/newsite/orders/new-order/
    You can see I have tabs for submit an order (which is a Jotform that works fine), New account (the troubled jotform), and contact us (sorry, it's not a jotform becasue it links directly into my client software).

    So if you use the form under the new accoutn tab, you will see my problems.

    first problem is that when I submit the form I'm taken to a captcha page.  However I do not have captcha installed on this form.

    Second problem is that it is supposed to post to a custom page so that the returned values get entered in to my database.  However instead I get a 404.  The page it should be posting the values to is http://www.chiseledimages.com/misc/newsite/account-return.  What's even more interesting is that when you submit the form, get the 404 error, and then copy and paste the URL that is displayed in the address bar, the page will load up fine (of course without the posted values), so the page does exists. 
    Like I mentioned I'm doing the smae setup for the form under the submit order tab, and it works fine. 
    I've tried using the jotForm plugin and shortcode, the script given by going to Embedd->Wordpress, and now the full source code.  All three give me the same results.

    Hi,

    That is a common issue when the form conflicts in your existing website scripts. You have jQuery libraries used in your site which conflicts the prototype scripts used in the form since you are embedding your forms as source code. The best way to fix that is to re-embed your form using iframe embed code. Since the form loads in iframe, it won't conflict anything on your page. How to get Iframe Embed Code
