Hello,
I created a form and added a start date and time for it to send to all employee each day . The form did not go out as expected. Can you please help and let me know why?

I have noticed that you have added a single recipient in the reminder email. You can add a total of 5 recipient emails on the reminder emails:
We recommend you to assign the form to the users. This way they can sign up to a JotForm account and can find the form in their Assigned form list. They can fill up the form daily:
