Up until a few days ago I was able to link to 5 forms at: http://www.jotform.com/user.php?page=myforms&formID=10823021261 for over a year.
Now I get an error message - "Not Found - The requested URL "user.php" was not found on this server"
I relogged on to Jot Forms and only 3 of the 5 forms are now in "My Forms". All the submissions are old so I tested the form on my site. The forms on my 3 websites seem to go nowhere when filled out even though I get a thank you message for submitting and the forms seem like they are being filled out there is no record in "My Forms" of the current submissions.

I have an unpaod subscription. I am willing to upgrade if that is what I have to do but I would really like to find the forms that were created for the 3 websites. Thanks you for your help. Best, Joan Lamensdorf

I'm having the same problem. My forms have disappeared and I'm getting a 102: server refused the request message and a 118: server timed out.
Jotform, are you there? Sent you an urgent message about it. Help!

I looked into your account and found only 3 forms. I confirm that the two other forms on your websites are missing and as such, I have submitted a report ticket for this issue. Our development team will take a look into this. We will notify you of any progress via this thread. Thank you for your patience.
