With the Forms tab selected one of the forms appears greyed out. I am using the Free version and thes form had reached 10 submissions. I have deleted these submissions however.
Do I need to reset the form and if so how?

The form is currently disabled. To re-enable it, go to the Preferences and on the General Settings tab, change the status to Enabled:
Close the settings and save the form when done.
Incidentally, you have set that form to receive a maximum of 20 submissions:
