What I need is the opportunity to allow visitors to the site to send in contributions in the following format:
Obviously, sender's name and emai address, then boxes for each of the following, ideally all in a line:
Year Name of Pub First/Last name Address City County Source Comments
There should be 5 lines of this information
Could the results be sent comma-delimited?

You can download the form submissions into a CSV file format.

If its the fields's alignment in the form, you probably were trying to do this Form-Field-Positioning
Fields positioned in a single line.
