I did upgrade my form "tocá y ganá con SuperBonder" from Premium to Professional, I need help with this because the form is disable even when I upgraded it.Page URL:

You have multiple issues with your form.
First, your form was disabled because it went over the limit of 1000 submissions per month for premium accounts. Your form will be automatically be restored when your submission counter refreshes on the next month.
Second, your form received a lot of multiple entries from the same user. I am not sure as to why that happened but it appears that you did not configure your form to accept unique submissions only, that is why you're getting multiple submissions from a single person. To learn how to configure this, please visit this page.
Also, I see that the required fields in your form do not get validated as required, thus allowing a person to submit without even entering any information. This happens because your form is embedded using FBML, which is already deprecated and not supported by Facebook.

I have reactivated your form and reset your submissions counter to 317, which is the number of unique submissions in your form.
Please do the steps I mentioned above to prevent duplicate entries. I cannot reset your submissions counter again.
