Is it possible to change the 'Country' drop down field in the Address Group to other drop down text options? Ideally I would like to have 2 - 'Month' and 'Year' - drop downs in replce of 'Country'. I know it's a bit of a stretch but the Address Field Group looks so much more concise and could work perefectly for inputting credit card details.

First, may we know the purpose of your form? You are manually asking sensitive CC information's in your form which isn't allowed here in JotForm. We have available payment tools that can do the job. Since they are integrated with payment gateways, CC payments will be more securely processed. You may try Paypal Pro, Stripe and Authorize.net all under Payment Tools.
We'd highly recommend using these tools as it's more legit and secure rather than asking this manually which may possibly trigger our phishing filter due to manually asking of CC information and there could be a higher chance that your account will be suspended.

Thanks for the prompt response. We're based in Australia and our payment gateway, eway, is not available as an integrated payment gateway. We are hoping to collect our customers CC details so we may then manually input them into our accounting system.
The form we are creating will be a SSL form, and we will only be allowing access to this form via a direct email to our customers. The info (CC or otherwise) would not be shared with anyone outside of our company.
If it is still too risky then we are happy not to ask for this info, but would like to know an alternative method that allows to ask for the form responder to decide from multiple purchasing options.

Even if you use SSL, it would inappropriate to ask for Credit card details online.
I am not sure if this is a solution for you. Authorization-only is now available for Paypal Standard and Authorize.net Integrations. However, in the case of Paypal payment capture can only be done manually, via Paypal's account interface. (You can read more about authorize-only transaction here.)
To enable this setting on either Authorize.net or Paypal Standard integration:
This way, you will avoid that our Phishing detector marks your form as a phishing one, putting in risk your whole account

Thanks for the answers - we'll remove the fields.
It would be great if Jotform would accept multiple payment options, that way the respondent has a choice of how they want to pay for a service.
