Currently I am paying for a form. I need to unsubscribe-please have someone from support contact me. Also, the form I created is not showing up under 'myforms.' I cannot view my reports since I can't see the form.

About your second question about being unable to see your form, let me tell you that I checked your account and I was able to see these forms:
If the form that you cannot see appears in the screenshot, I will kindly ask you to clear your browser cache and cookies and then log back in Jotform.
