    We apologize for inconvenience. I reviewed your account/username selfcare.bestcare365  and I noted that it was suspended due to the suspicious fake loan offer/scam form on the account. The form violates the Terms of Use.

    We can only ask for reconsideration to activate the account after you comply to the terms and agree to have the suspected form/s deleted on the account.

    Why was it flagged as fake/ scam ? I was creating an application form for a
    LEGITIMATE loan company in Jamaica.
    Can you delete the offending form so I can regain access and retrieve some
    info? Thats all I want at this point.

    Your account is suspended due to fake loan offer/scam. Can you please give us more information that you are associated with a company/store?

    We understand this might not be the case with your form, yet we had to act according to the general rule to be in compliance with our Terms: Free accounts with non-corporate emails showing suspicious forms must be suspended immediately. 

    In order to reactivate your account, kindly provide us with the following information:

    1. The URL where your forms are used.

    2. Your website or proof of affiliation to a legitimate institution.

    We also need you to change your registered email to a corporate one to avoid getting flagged again by our phishing detector.

    Send these proofs to reviewers@jotform.com with Thread #2477368 as your subject. We will let you know about the status of your account after a thorough review of your proofs.

    Or, we can just delete the offending form and lift the suspension of your account.

    1. I JUST started putting the form together so I haven't started using it.
    2. I do not want a corporate email.
    3. I already asked for that particular form to be deleted so I can access
    my other forms
    4. How can you bar me without even notifying me?

    We have now deleted the offending form and reactivated your account.
