Hi,
You can create a form by following this guide: https://www.jotform.com/help/2-How-to-Create-Your-First-Web-Form
After that, once the form is filled out, you can download the PDF form by following this guide: https://www.jotform.com/help/73-How-to-Download-Form-Submissions-as-Excel-CSV-PDF
If I misunderstood your question, please let us know. Thank you!

Hi Vincent,
My question was can you create an online form that has as an example a name, address, date of birth and other information and then use that data to populate another form that happens to be a PDF and then have the completed pdf returned to the form filler for review?

Thank you for providing more information about the question.

I created a simple demo form and submitted it with 3 different PDF documents but the same Name and Email. Here's a screencast:
If you want to email back the PDF document to the submitter, you can add an autoresponder email to your form. In this way, the form submitter will receive a copy of the submission or PDF document.
If you're referring to the Fillable PDF, unfortunately, you can only populate one Fillable PDF. Please check this guide for more information: https://www.jotform.com/help/71-Prepopulating-Fields-to-Your-JotForm-via-URL-Parameters
