Your guide seems to be outdated. I have attempted to follow the instructions the best I can. Setting up the App however I have had to take the app out of sandbox to allow JotForm access.
On the "Select type of information you'd like to collect screen" there are 10 tick boxes 6 of which are automatically highlight although there is no field name next to them.
If you continue to the next stage you can adjust the Facebook log in notice section just fine.
I have edited the form correctly http://www.jotform.com//?formID=32052376050949
However when you preview the code or embed the code an error screen appears reading: "Invalid Form URLThis form is not available for this domain.
I have attempted to embed the form on a page on our website that is rarily used; temporarily on this page http://greatgrass.co/environment but ideally the form would be going on the testimonial pagePage URL:

We do have domains separation rule on forms. Please make sure to use the correct domain for your form. Your form domains should belong to www.jotformpro.com since you are a Premium user. e.g. http://www.jotformpro.com/form/32052376050949
Here's a tutorial on how to get the FB Integration working. http://www.jotform.me/help/149-Facebook-Forms-Integration-Video. Just make sure the correct Site URL is entered in your FB App settings.
Let us know if you still have any difficulties so we can assist you further.
Thanks and Regards!

I think I've finally figured the thing! Great for easy data collection
Is there any way of using this facility as to share it with the Facebook users friends, such as intergrating opengraph features.
I'll give you an example:
Customer submits a review of our service via a JotForm; they log in via Facebook (collect basic details as it does now but also requests open graph permissions to publish) and then they submit.
When they submit the review would appear in users Facebook feed.
Any suggestions? Can it already be done? If not is it something that could be added as an idea.

Thank you for updating us. We're glad to know that all is sorted out.
As for your new inquiry, I suggest you instead create a new topic for it, and provide us more details about the opengraph requirement you wanted to do.
You can create your new thread using this link http://www.jotform.com/contact
We will attend to you as soon as we can.
