I've check your form and found out that the issue is had something to do with your form's email notification. I think you made some changes/update with your form which causing the issue. To fixed the issue, you need to re-create your form's notification.
Note: If you make some changes/updates with your form, you need to either manually update your email notification or re-create it cause it won't automatically update once you made some changes like adding or deleting some fields on your form.
