PDF attachment not displaying every question

    The patient fills in the online paperwork, their questions and answers get sent to our reception team in the main body of the email as well as a PDF attachment.

    Why isn't the PDF displaying every single question and answer of the form?? 

    Please see example attached.

    Hi,

    we apologize for the inconvenience. I have cloned your form and tested it, and the functionality seems to work properly:

    I would suggest that you reset the layout of the PDF by going into the PDF Editor and creating a new document:

    You can then choose this new document as an attachment to your email notification. Please give it a go and let us know if it solves the issue.
