When If/Do conditions are used, I am unable to see the fields I want

I have many of these types of statements where based on the user selection, a particular field appears. The only one that seems to work are the first two. When I try the other ones, the incorrect field appears. I have checked to make sure my "do" statements are showing the correct fields. But I am unable to get EULA field to show when I select EULA response from my first question. 

This is the question that I am using for my conditions.

If Access request > show Access 

If Alert/Notification > show Alert/Notification

If EULA> show EULA

When I select more than one, it seems to work only for the first 3 responses. If I choose any single response it doesn't map to the correct field.
