Form Submit freezes

I have just created a form, used the QR code to generate it on my phone, then completed it to test it, but when I press 'submit' it freezes for some reason??

Are you trying to generate QR code with this approach? 

Are you embedding the form to any webpage?

Also please try filling your form using other browsers and devices. Hope it helps . If not then feel free to contact.
