My log in isn't working

The website says my form has been disabled and I cannot log in with my email anymore. Please advise

Upon checking, your form https://www.jotform.com/form/201867403785058 has a multiple violations.

1). Asking for username and password

2). Asking for SSN and other sensitive information

As per our terms of use https://www.jotform.com/help/8-Terms-of-Use:

Collecting Sensitive Information. You may not use the JotForm Service to collect certain types of sensitive information, including but not limited to, credit card information and any type of login credentials. You may collect some sensitive information such as social security numbers or driver’s license numbers, but you are required to use best security practices of JotForm, Inc. including SSL and Encrypted Forms features. You are solely responsible for compliance with any data protection and privacy laws and rules applicable to the sensitive information.

For SSN and other sensitive information, you need to comply with two things.

1. Add Terms of use widget (for your users to read and agree). This will surely make your forms not to be considered as guilty of phishing activities.

2. Add a Signature widget where your users sign and agree to provide such sensitive information.

In order for us to activate your account, we will need to delete the offending fields. Please read our terms and confirm by replying here that you agree.
