I have a form that we are requesting login credentials. I understand this violates the terms. My question is, as sensitive information that can be acquired like SSN as long as the form is encrypted, is it possible to continue utilizing my form as it is with the requested credentials?

As of this paragraph from Terms and Conditions

Collecting Sensitive Information. You may not use the JotForm Service to collect certain types of sensitive information, including but not limited to, credit card information and any type of login credentials. You may collect some sensitive information such as social security numbers or driver’s license numbers, but you are required to use best security practices of JotForm Inc. including SSL and Encrypted Forms features. If you are asking for WIFI password then you can , and its ok since its just a password of a WIFI, other than that you cannot collect and our Phishing detector might suspend your account if so .

Also put a Signature Widget in your form for more validity.
