    Thank you for your message.

    In this case, the account has been suspended because the form is asking for credit card information, this is not allowed by our terms of service since the users can´t save the CC information in the submission dashboard.

    In this case, all our forms are secured https://www.jotform.com/security/ but to avoid frauds and collect payment without the user authorization.

    You can just collect payments using the Payment Integrators: https://www.jotform.com/integrations/category/payment-processing.

    Noted, can reactivate the account so I'll remove the credit card option
    because my clients are telling that the form isn't reachable.

    We also need to verify your company/business, for that reason you need to send proof of legit business: In order to reactivate your account, we will have to delete the offending form.

    1. The URL where your form is used.

    2. Your website or proof of affiliation to a legitimate institution.
