Set up Order Form and Embed to Website

Hello, My name is John. I currenley have a website that has created a link with the payform for pay pal. My web-site consist of custom made T-Shirts. I need tis form to include color and T-shirt size, whitch I actually need help with. I would also like to know how may I get an embeded code so that I can encorperate it to my website.
