Get a new BAA

Hello,

I was informed by my our legal team that we will need to execute a new BAA upon their approval. Would it be to send me a copy of the BAA that can be completed and sent back to JotFrom? This account is connected with a university, and we need certain approvals.

When you complete HIPAA upgrading you should then get a BAA form. If you've not seen it you can request it on this page (there's also a sample agreement), and you can then send this to your legal team.

Thank you for the quick reply. I sent my department the sample BAA. This was the response I received back

"The sample BAA that you provided does not have Temple’s information or any date’s. Can you please ask JotForm for a version of the one that you signed, without your signature, that shows the agreement is between JotForm and Temple Athletics with the effective date? Since you are not an authorized signature, that contract you signed is invalid."

It looks like a member of the legal team will need to sign a new BAA for the account. What is the best way in setting this upaup sending to the appropriate parties?

You can request a new BAA through this page https://www.jotform.com/hipaa/baa/.

Once you get the real BAA, you should see the real information like the name of the company, date and etc. in the following parameters.
