On Editor for pdf to form I did upload pdf and it was working fine but suddenly it was showing something went wrong and then reload and after that preview of pdf on right side gone.... so please give me a solution..as soon as possible.

Hi,
thank you for reaching out to us. We apologize for the inconvenience. In can see that your form "Patient Information Form" is created from a PDF, however, the other form "Medical History" is appearing as if it were created from scratch.
Since we don't have access to HIPAA forms, can you please tell us the exact steps you have taken to create this form (https://www.jotform.com/form/202144043177042) in order to help us in testing this on our end?

Hi,
I did another form https://hipaa.jotform.com/build/202220501077035 and same problem again... when I was in the middle of that form .. suddenly lost connection of pdf. on right side pdf not showing...

Hello there,
One form is a Form Builder where there is no PDF by default and it consists of Form elements.(Medical History)
Another one is a PDF Editor with integrated PDF (original) and form.(Patient Information System)

Since they are both different Builder Modes in Jotform will show different layout.

It was same as my other form but somehow pdf gone and on top of left showing form editor.. but first time it was pdf converter..

Hi! I do see a "Preview PDF" button at the bottom of your form https://hipaa.jotform.com/form/202220501077035 which is only displayed for the forms that are converted via PDF (screenshot attached below). Also, this button does not work.
So the problem does exist. Please answer the following questions:
Q1) Is there any particular field that you believe working on which breaks the connection?
Q2) Sometimnes, browser extensions interfere with the page's working which can also result in this issue. Can you try re-building the form in Incognito mode (Incognito windows have browser extensions disabled by default)?We'll wait for your response.
