How do I create a point sytem?

What Widget do I require to get an overrall point system from the form I am trying to create.

ie - Unsuccessful = 0 points, Successful = 1 point, Excellent = 3 points? 

Then at the end of the report a number would be generated out of say 100 points? So the person filling in the form might get 56 points over all?
