Why was my account disabled?

It seems your form 202192293999165  was reported by our phishing detection system as suspicious of phishing activities. It seems your form is collecting uploads of Driver's license and banking information.

Due to our PCI certification, we are required to deny the collection of credit card/bank account details on  form fields. https://www.jotform.com/blog/pci-dss-service-provider-level-i-compliant/

In order to lift the suspension in your account, we would have to delete the guilty fields.

It seems you have no submissions yet in your form, I have removed the fields and reinstated your account.

 Please note that it is possible to collect Driver's license number,If your purpose is not to collect these data from the general public, but from your customers, then you need to comply with two things:

1. Add the Terms & Conditions Widget (or the Short Scrollable Terms Widget) for your users to read and agree.

2. Add a Signature Widget so your users can sign and agree to provide such sensitive information.

3. Otherwise, you must enable form encryption on your form if you need to gather critical personal information
