    It seems that our Auto Phishing Detector might have seen your form as a threat to our Terms and Conditions.

    Though you are not collecting credit card number due to a field name including the word "Credit" in it the problem might have persisted.

    You can remove the field that includes the name "Credit" and replace it with something else.
