    Hi,

I have checked your form and I can see that you are currently using the Authorize.Net integration. Since you want to add a payment check option you may add a Single Choice field, to let your users choose which payment method they want to process their payment then through conditional logic the payment fields will show based on the user's answer.

You may refer to this detailed guide, although it uses Paypal the workflow is similar to your requirements:  How-to-Set-up-PayPal-Along-with-Other-Payment-Options

I hope this helps. Let us know if you need further assistance.

Thank you for the suggestion. But I still can’t get it to show option for credit card and option for Echeck. It won’t let me pull in two payment gateways. I believe authorize.net does both credit card and echeck.

    Hello Diane,

Unfortunately, it is not possible to have multiple payment gateways in a form, so the workaround is to provide the form users instructions on how to process their payment via ACH.

Here's a demo form to further assist you: https://form.jotform.com/202198033266958

However, there's another workaround and that would be combining two forms via the iFrame Embed widget, with different payment method please refer to this detailed guide: How-to-Create-a-Basic-Multiple-Payment-Form
