terms in my form says "type 1 option"

my clients click the option which is a round circle I added to the bottom of my page, in place of a signature. Why when i recieve the submission at the bottom it says - Type option 1

this is an important document. why does it say this when client clicks this button?

I believe this is the question you're referring to.

The reason why it only displays is that you used the Single Choice field for the Terms of your form. I would suggest using either the Terms & Condition Widget or Short Scrollable Terms Widget instead. See the sample below:
