Hi Ajay,
Your account has forms in for loan applications. These are only allowed where the account account belongs to an authorized bank or credit union.
I can see there are submissions on the forms, so would need your permission to delete the submission data & forms. If you provide this then I will be able to re-activate the account.
If you would like to have loan application forms after your account has been re-activated you would need to send the following to reviewers@jotform.com, with the thread ID (2500791) in the subject line:
- The URL where the form will be used
- Proof of affiliation to an authorized bank or credit union
- Any additional documentation that will help our team process your requestLet us know if you have any questions.

Hi there,
Once our Reviews department has verified all your information and you get their approval to use loan forms than your account will not get suspended again.
In case if the automated system does suspend it for some reason we can activate it again.

Greetings.
If your account gets suspended again, our reviewers team will activate it. However, once your account gets unsuspended, it should not get suspended again.

Kindly send them to reviews@jotform.com. They will be the ones reactivating you account once verified.
