What will be best way to do a form like this?

Yes it is possible to build form to achieve your requirement. Here are the steps you need to follow:

#1. Add single choice question in form for each question. 

#2. Add all the 5 options in each single choice question.

#3. Set the calculation value for each option which will be used in calculate the average score.  The following guide should help you how to set calculation values: https://www.jotform.com/help/301-How-to-Assign-Calculation-Value
