I'm not able to log into my account and my clients aren't able to access the forms. Thank you in advance for you help.

Our reviewer team suspended your account because one of your forms is collecting credit card information. This is against our terms of use: https://www.jotform.com/terms/
We need to delete the form so we can activate your account.
If you want to get payment authorization, you can do that by following this guide: https://www.jotform.com/help/292-How-to-Enable-Payment-Authorization

do this because some of my business is online telehealth and I need to have
cc info on file.

Sorry for the inconvenience.
Unfortunately, that is still not allowed even if you're a subscriber. However, you can ask or collect for an email address or phone number and then ask them directly about the credit card information.

You may delete the cc field to prevent your account being suspended again.
