This form ends up with wacky formatting and missing elements when converted to pdf fillable form--I've tried about a dozen times, each times produces slightly different pdf with slightly different problems:

In this case, I download a PDF from your form you can see it here: https://drive.google.com/file/d/1wndBFFS0BstGsFIofsMKpT_6X3126SJo/view?usp=sharing Seems like the issue ha been resolver, I check the Singe Choice field and at the moment just have the, Yes and No answers, I also clear the cache in your account.

Thank you for taking the time to download and reply but not sure why you think it's been solved. In your own Google doc, there are at least 8 mismatched or missing items in the first 4 pages and the last page Deons't match the online version either. I didn't look through all 30 pages but this has been my experience with over a dozen tries--each one missing or moving a question around so that the pdf version is nonsensical and doesn't match the online version. Can you see that pages 1-5 are not exactly the same? They all have some questions missing, or showing up randomly 12 pages later when they don't belong.

Attaching screenshots of the same question on the online version versus your google doc. They don't appear in the same place and there are about 20 instances of the same kind of error for other questions. The questions are getting jumbled when downloaded as a pdf.

Thank you for your response.
In this case, after checking your form I create a clone and I have the same wrong format in the PDF, so there are 2 options to resolve the issues.
1. You can fix the position moving the fields manually:
2. Or, create a new Fileable PDF and the system is going to create the PDF with the right positions of the fields:
