Are you referring to this form?

May we know if you have embedded this form on any website or the user is unable to fill using the direct link?

    Looking forward to your response.

    Thanks.

May we know how the users are filling out the form? Is it via the mobile device or via the desktop? Is it embedded on any website?

Can we test submit your form?

    Looking for further information.

    Thanks.

Yes, test it all you want to. I know that one of the person's used a desktop and the others I am not sure what they are using, but it was never an issue before, now suddenly it is, but they say the form shows up then dissapears, and when I tell people about it then they come back admitting there is a problem, that's all I know.

    Hello there,

    Thank you for reaching out.

Since the issue is happening on an embedded form, please share with us the URL of the page where the form is embedded.

If applicable, you can send us screenshots to better illustrate the issue. -How to Post Screenshots to Our Support Forum.

You can also send us screenshots of the console log if it shows any errors. -How to get a console report from most common browsers.

I've embedded your form onto a test website, but wasn't able to encounter the issue. I was able to fill the form without a problem.

What embed method have you used(iframe, JavaScript, full source code, etc.)?

Try embedding the form using a different method, see if the problem persists. -Which Form Embed Code Should I Use.

We'll be waiting for your reply.

I've just made a test submission to the form embedded on your website and it submits fine as expected. So it could be related to the user's browser. Please advise them to clear their browser's caches before accessing the form and see if that makes any difference.
