    Thanks for the response. So i am looking through the templates to see if I can find what I need maybe you can point me in the right direction.

    I want to create a form that will be like a self assessment, the goal is to find out which areas of Dressage riding the weak at or strong at, at the end of the quiz they should be presented with the result, something along the lines of "based on your answers, you need to improve on these areas"

    Then we make a recommendation on the best way to improve that area.

    Similarly we can commend the area's the person is strong in. Let me know the best way to do this.

    Yes it is possible to achieve your requirement by adding conditions and "Form Calculation" widget to calculate the score and show the recommendation. 

    I would suggest you to please share an example with few questions along with how the score should be calculated and based on score what recommendation should be displayed, we will build a demo form for you.

    Our question sheet looked like this;

    What are your strengths and weaknesses as a Dressage

    Rider?

    This assessment will help you identify strengths and

    weaknesses in your riding and to understand what areas you

    need to focus on to make progress in your riding. It’s ok to

    be unsatisfied with some of your answers. That is the point

    of this exercise! It is an indication to focus on that area and

    set some goals. Improvement and change are hard and take

    time so don’t get discouraged and persist in your journey to

    the next level of potential and performance!

    Rider Position

    In general my riding position is correct. I maintain proper

    alignment from my shoulder, hip, and heel at all 3 gaits and I

    am able to stay with the motion of the horse without

    bouncing.

    Mission

    I have a clear plan and goals with my horses and my riding. I

    know my strengths and weaknesses and I have an idea of

    how to work though these things. When I look back to a

    year ago, I have made progress in my riding.

    Learning

    I have good teachers and mentors that I can ask for help and

    guidance from with my horse. If I run into a problem, I have

    resources to help me and my horse understand. I feel like I

    am making progress (it can be slow) and that me and my

    horse understand the training.

    Health

    I take care of my own health and fitness. I am fit and healthy

    and take care of my physical and emotional health so that I

    feel energetic and strong everyday. I eat well, sleep well,

    and exercise so that I can ride and take care of my horse.

    Friends

    I have a good group of friends that encourage and support

    me on my horse journey. I know that I have support and

    encouragement during the highs and lows of riding and

    training. I offer support and encouragement to others with

    their horse journey.

    Horse Power 

    I have a horse that is suitable for me and my goals. I am not

    “over-mounted” with a horse that is too much for my skillset

    nor am I “under-mounted” with a horse that is not physically

    capable of doing what I want. I can ride the horse I have and

    can progress with this horse.

    Symmetry

    I feel very symmetrical and even on my horse left and right. I

    have even contact on my left and my right rein and I am

    equally comfortable riding a left circles as I am riding right

    circles. My horse move easily and equally off of both my

    right and my left leg.

    Confidence

    I feel confident handling my horse in new situations and on

    days when he/she is fresh. I have the tools and skills and

    enough of a relationship with my horse that I know I can

    work through these difficult situations. I can be independent

    in handling and riding my horse.

    Knowledge

    I am knowledgeable and experienced with horses and with

    training dressage horses. I have a good sense of how to

    communicate and train a horse. I understand how to

    implement the principles of the Dressage Training Scale and

    to use it as a framework to develop my horse.

    Passion

    I have a passion and commitment to riding and horses.

    People think I am horse crazy! No matter what, I make it a

    priority to get the the barn and to ride my horse. There are

    no excuses. I make it to the barn nearly every day.

    Persistance and Perseverance

    I know that becoming a good rider and horseman is hard

    work. There will be highs and lows and I am prepared to

    persevere and stay committed during the good times and the

    bad times. I am in it for the long haul and am prepared to

    enjoy the journey as much as the destination.

    You can use a single choice field, assigned calculation values to the options, and use the form calculation widget to calculate the average to achieve this.

    I created the form, having some issues with the results part tho. The form can be seen here --> https://form.jotform.com/202255385091857

    I want to display the results showing area's that the person are week in which would be if they selected a value less than 5. SO i want to display the results like this.


    Based on your answers you need to improve on;

    Rider Position
    Friends
    Horse Power

    My advice to get better at these are as follows.....

    You seem to be competent in these areas; (Where i would put the ones they listed with a value greater than 5)


    Mission
    confidence
    etc


    That's what i would want the result to look like, but how do i record the ones they answered less than 5 in and the ones they selected more than five, can you assist.

    Hello,

    Thanks for your patience. 

    I found a way to achieve this through Autoresponder.

    Here are the steps.

    1 - Add a Form calculation widget for each question

    Add a formula that will pass star rating values to the calculation widget.

    2- Add two "Short Text " field for each question 

    One that will be filled in case rate is less or equal to 5, and another one that will be filled if the value is greater than 5.

    3- User conditional logic to pass the question one of the text fields based on the value returned value present on the calculation field.

    4- Create a custom Autoresponder with the text fields.

    Thank you for this, took some time but I was able to figure out how this works and modify it to include all questions. Thanks alot again, my problem has been solved and this ticket can be closed.
