I cannot access my form on this page:
I had someone else embed the form, and he did it in some fashion that
I cannot change it. And I cannot get a hold of the guy to do so.
I looked in my past emails with him, and have determined that this is the form code:
But when I attempt to embed it or clone it, it doesn't work.
Please advise. Thank you.

I just checked your form and found that the form http://www.jotform.us/form/31559558392163 was created in a guest account. That's why you can't access to edit your form. We can transfered that form to your original account, but first I want you to confirm us if kungo is the account where you want to have the form.
Also let me tell you that the form embeded in the page http://aenys.com/test.htm, which is this http://www.jotformpro.com/form/32098489732971 is not the same that is in the guest accoun which is this http://www.jotform.us/form/31559558392163. So if you have a problem with that form cause may be another.
Please confirm us the account to transferd the form that is in the guest account.

The name of the form is AE. I have double checked in your accoount and your form is there, please look:
That was the form I told you that was in the guest account.
If you still don't see it, please log out Jotform, clear your Browser's cache and cookies, then log back in Jotform and check if you can see the form.
