If you want to use your custom domain name like mycompanydomain.com (so that the URL looks like this http://forms.mycompanydomain.com/1234567890) that's only possible with JotForm Enterprise:

https://www.jotform.com/enterprise/

However, all the accounts can create a custom URL of the form. That is, you can add your own form name at the end of the URL along with your Jotform Username, like this below:

https://form.jotform.com/Niko_N/demoform
