Making our ECAP a Fillable Form

Hello!

I am an academic advisor for a district in Phoenix, Arizona. Now that we are switching to virtual learning because of COVID19, there are some important documents we need to make into fillable forms for our students. I attached the two documents, and honestly, I don't even know where to begin. If I just want to convert these two documents and be done, do I really need to pay a monthly fee? Do you have any discounts for educators?
