Incorrect Form Submission Download

    In my JotForm account (Silver).  I have made multiple forms for businesses which I am the medical director.  

    I would like you to look at these two downloads from a submission on the NCHH ONLINE Registration Form.  

    In the inbox, there are more than four ways to download a form.  I am not sure why, but there are none the less, multiple ways to download a form and they all generate different versions of the form.

    While I see the benefit of generating a fillable pdf, shouldn't the content on the form always be the same?  

    When I downloaded one way (part of the form is a section from a completely different form which I have made for a different company... NSTMS new patient form) but when I download it another, the copy is the correct copy.... Why does it do this?

    Incorrect version with a paragraph from another form inserted???

    It is important to know that we like this second way to download as it shows the patient's insurance cards and a nice header with the logo on the top.  The insurance cards are visible without a second download of them individually, they are bigger, more legible way... but the form is wrong with this copy.  I wonder if because I used the NSTMS company new patient form and cloned it to make the new NCHH form if something is stuck in the code somehow and inserting the wrong copy with one of these 4 ways to download.

    Hi @drcochran,

    Thank you for reporting this! Can you just confirm which of the 4 options you downloaded the correct one from, and which you got the one with the cloned field from? 

    Also, what do the other two options generate? 

    And finally, if you go to edit each form and at the top click 'Revision History' what do you see there for the form's revision history?

    While I appreciate you asking, I have already wasted enough time on this process, I feel like it your responsibility to test the software.  I really feel like I have told you plenty to identify one of the issues.  It is a software bug for sure.  I really don't have time to fix your software or to go back in and test each option for you and report it.  

    #3 option to download, above is the only download option we are using now because the others all leave things out (images, etc) or insert wrong information.  

    Frankly we are not even doing that as we have integrated our JOTFORM submissions into our HIPAA compliant Google suite (thank you for that option).

    We are sorry for the inconvenience this has caused.

    We will surely test each of the Inbox functionalities in our test HIPAA account to see if there are discrepancies in each of the download options and report our development team regarding this.

    I was testing the cloned version of the form and noticed there are two types of PDF that could be downloaded from the 4 options that you have mentioned. One is the Print Version which excludes the texts that is present on the form and the other one that has all the fields including the texts.

    Did you get the pop-up to update the PDF when you checked the PDF editor? I believe you have cloned this form and edited it. It seems the Paragraph is not updated in your current PDF. You need to create a new PDF to get all the fields updated on it.

    Thanks. I don’t need a pdf form. I need the online form printed as a pdf

    Yes, it would download as PDF-Document.

    In PDF editor you have two documents, one PDF Form and another PDF Document that needs rectification as it is outdated and the same PDF is downloaded from the Inbox page. You need to rectify this document in order to get the correct PDF while downloading and printing it.
