Creating HIPAA Form With Conditional Confirmation Email

    Hi,

    I need to re-create this Formstack form in a HIPAA-compliant environment. The form collects answers to three questions and, based on the answers, will send either a green "entry permitted" or a red "entry denied" email to the user.

    Does JotForm have this ability? If so, we would be interested in starting an account.

    Hello Ryan - That is possible by creating two autoresponder emails, one for permitted and the other's for denied. Here's an example:
