    To clarify, are you referring to the a JotForm account?

    I have checked your email address however, I can't seem to pull an account.

    Kindly share your account username so that we can check?

    It seems your account cellisfitnesscenter was auto suspended by our phishing detector due to the logo you have added in your form.

    We understand this might not be the case with your form, yet we had to act according to the general rule to be in compliance with our Terms: Free accounts with non-corporate emails showing suspicious forms must be suspended immediately.

    In line with this, we would like to request these information from you:
    1. The URL where your forms are used.
    2. Your website or proof of affiliation to a legitimate institution.
    3. Share with us a document that will prove that you're authorized to use the said logos.

    We also need you to change your registered email to a corporate one to avoid being flagged again by our phishing detector that can lead to another account suspension.
    Send these proofs to reviewers@jotform.com with ticket#2509636 as your subject.

    We will let you know about the status of your account after a thorough review of your proofs.
    Or, we can just remove the logo and lift the suspension on your account.

    Could you just remove the logo?
    A simple google search will tell you it’s a legitimate business that I own.
