Employee Evaluation Form for Multiple Supervisors

    I want to create a team member evaluation form to be used by our supervisors to evaluate an employee. The employee will be evaluated by multiple people. Is there a way to have one form that can move from supervisor to supervisor and they each fill out the evaluation and then when complete, we have one form that can be printed/reviewed?

    So this appears to allow the next person to "approve" or "decline" the submission but is there a way for each person to add to the submission.  Basically we have an employees that will receive the bi-annual evaluation.  Each employee is evaluated by 4-7 supervisors.  We would like all the supervisors evaluations to be on ONE form.

    Could I use the workflow to allow the next person to complete the evaluation form?  It seems like it would be alot but maybe duplicate the form 7 times and hide the other ones based on which step it is on?  I don't know, I am just talking it through.

    In the end, I would create a PDF form with each evaluation question and then each supervisor's answers under it.

    Let me know your thoughts.  Is it too much for me to do in Jotform?

    Hi there,

    It's tedious, but it doable. 

    Start off by adding all the fields that are to be filled out and arrange them according to the workflow.

    - Using the Get Page URL widget, you can create conditions on top of using this field to specify which field a submitter will see.

    In the end, I would create a PDF form with each evaluation question and then each supervisor's answers under it.

    YIKES… hahaha
    The attorneys are like I have seen Survey Monkey do surveys and the consolidate all the answers into one.. That is kind of what they are looking for. Of course, I wanted to do in Jotform cause that is my go to.
