JotForm page loads incorrectly on mobile

I've just tried here in Chrome & Safari on iPhone and the form loads normally, can you let me know which browser you are using?

And also, have you disabled Javascript in Safari?

Hi thank you for your email, I have tried again using the click on
Instagram sometimes it does load well, but it would be better if it did all
the time, of course, as it may deter people from using the product.

Please note that all forms are mobile responsive and you should not have any trouble since you are using form's direct URL. 

I would suggest you to please test your form again and get back to us if the issue persists.
