Dates not showing

Hello- 

On one of our forms, we received two submissions where the "date select" feature is not printing on the PDF -- its not even looking like people had to submit a date, despite it being required. 

When I fill out the form using the "preview mode" and "fill form" feature, the date does populate on the PDF and prints accordingly. 

Can someone help with this?
