First page of my form has disappeared

I went to look at https://eu.jotform.com/build/83006426081348 and was going to copy some text from the front page. I don't know what happened but it has disappeared!

I looked at the revisions and it did look as if I had made a change. I reverted this but page one is still missing. It is a page of conditions and contacts and has no boxes to be filled in. Part 2 of the form is there still.

Can you restore this please?
